Telehealth

 

Our Telehealth program is an innovative healthcare delivery program that utilizes technological advances to overcome barriers to care. Veteran patients with chronic diseases such as diabetes, heart failure and chronic pulmonary disease can be monitored at home using home Telehealth technologies. This prevents or delays an elderly Veteran needing to leave their home and move into long-term institutional care unnecessarily.
