Joint Commision Statement
You have the right to present concerns pertaining to patient care and safety at all Health Care System locations including Outpatient Clinics. These concerns should be directed to the Manager of the Service or Department that is providing such care. If you are unsure of whom to contact, any employee can direct you to the appropriate supervisor. Should it appear that your concern is irresolvable
at this level, you may contact the Patient Advocate located in Rooms 7 or 8 on the first floor of the medical center or by calling 1-800-472-1365, ext. 4839 or 4854.
Any individual with a concern about patient/resident care and safety in the hospital should contact the hospital management. If your concern cannot be resolved through the facility, you may contact VA Healthcare - VISN 18 in writing at:
